Unfortunately, we are not in network due to lack of reimbursement for the amount of time we spend with our patients.  We do, however, accept CareCredit, Health Savings accounts and Flexible Spending accounts. Because of the extended time we spend with each patient, we have decided to dedicate our time to patient care versus insurance billing.  As such, full payment is expected upon online booking or at the end of each visit. However, we will provide you with a superbill containing diagnostic and treatment codes that you will need to submit to your insurance company to get some of your payment back according to your out-of-network policies.  Filling out an insurance claim form is very straightforward and should not take more than 10 minutes. If you have questions, print out the form prior to your appointment and we will assist you!

Functional Medicine Appointments:

  • New patient intakes are generally $750 and last up to 90 minutes including a physical exam and blood draw (extra fees for sedation)
  • Our hourly investment is $500.  Most follow up appointments after you receive your treatment plan are 30-minutes unless labs are being reviewed or the patient requires more time.

Well and Acute Care Appointments*:

  • New patient well care appointments last 60 minutes: $250*
  • Established well care appointments: $175*
  • Acute care appointments: $175

*Additional charges will be included for blood draws, strep/flu/COVID and other testing

Genetic Consultations*:

  • For test ($325-349) and 60-minute consultation with Dr. Monica and nutritional plan ($375)

Electrodermal Scanning:

  • In-office/distance scans and treatment plan with Dr. Monica are $225

Under Georgia law, patients must first be seen by a physician in person prior to having a telemedicine appointment via email, phone, or videoconference. However, once an initial appointment has occurred in person, future visits can be conducted virtually.

When you call and schedule an appointment, time is reserved especially for you and no one else. Since the appointments are much longer than standard office visits, cancellations are significant interruptions to the Practice. Thus, a minimum of 5 days’ notice is required for cancellations of new patient visits and 3 days’ notice for existing patients. There is a $200 non-refundable deposit for new patients. Additionally, there will be a 100% office visit charge for “no-shows” or late cancellations. We also require that all new patients complete and return their new patient paperwork no later than 3 days prior to your appointment. If the new patient paperwork is not received, we will reschedule the appointment and cancellation fees will be applied.
